В общем вопросе подлежащее и сказуемое меняются местами - так он образуется, а специальные вопросы начинаются с вопросительного слова, поэтому первый вопрос общий, второй специальный. В первом и четвертом предложении специальный вопрос - это вопрос к подлежащему, потому сохраняется прямой порядок слов.
